Overview of the Referral Process

1.   Referrals may be made by submitting an application online by clicking here
 and submitting a complete application packet.

2.   Youth must be referred by their legal county DFCS office, DFCS State Office of  Provider Management (OPM), or a DJJ Placement Specialist

a. For the referral to be OFFICIAL and for the youth to be reviewed, the MAAC DHS Referral Form must be signed by the County Director or their designee or an email from OPM must be reviewed.

b. DJJ Placement Specialists may send an email or fax to refer a youth.

3.  After the Intake Team reviews the information, MAAC will contact you for more information and schedule and interview with the youth.  MAAC may have multiple agencies interview/review a youth prior to a decision being made regarding placement.
